Discussing care goals with patients is often challenging, especially during a serious illness or toward the end of life. This webinar will review the kinds of conversations that are necessary for effective advance care planning and when to have them. Presenters will also discuss how to approach implementing advance care planning in an office setting and review emerging health care programs that highlight the need for quality advance planning conversations.

Lael Duncan, M.D., is a licensed California physician with more than 15 years of clinical practice in internal medicine, infectious disease and long-term acute care for patients with multi-system disease. She currently serves as CCCC’s Medical Director of Consulting Services. Before joining CCCC, Dr. Duncan was co-founder and medical director of Good Medicine Consult and Advocacy Services, PC, a practice in the San Francisco Bay Area focused on assisting patients and families with shared medical decisions and values-based care planning.
Leah Morris, R.N., MPH, M.S., N.P., is a registered nurse with more than 30 years of experience in health care. She currently serves as CCCC’s Director of Policy and Payor Relations. Morris has experience developing and implementing managed care programs for private and publically financed health insurance programs, and was part of the effort to bring Covered California to market.
